Supported Community Connections (SCC)

Building Skills, Creating Connections, and Strengthening Independence

Supported Community Connections services help individuals with intellectual and developmental disabilities participate more fully in their communities. At The Grand Company, we create personalized opportunities for individuals to explore their interests, develop practical skills, build relationships, and enjoy meaningful experiences outside the home.

Our goal is to help every person feel more confident, connected, and included while honoring their choices, abilities, preferences, and individual service plan.

What Are Supported Community Connections Services?

Supported Community Connections, commonly called SCC, is a person-centered service that supports individuals as they participate in everyday community life. Activities are selected based on each person’s interests, goals, abilities, cultural preferences, and desired level of independence.

Rather than simply accompanying someone into the community, our team uses each experience as an opportunity to strengthen skills, encourage decision-making, and support meaningful participation.

How We Can Help

SCC services may help individuals:

  • Build confidence in community settings
  • Develop social and communication skills
  • Form friendships and meaningful connections
  • Practice personal safety and situational awareness
  • Learn how to navigate familiar and unfamiliar places
  • Identify and use community resources
  • Improve decision-making and self-advocacy skills
  • Practice money management and purchasing skills
  • Strengthen time-management and planning abilities
  • Explore hobbies, interests, and recreational activities
  • Participate in volunteer opportunities
  • Increase independence with everyday activities
  • Develop healthy routines and wellness habits
  • Reduce loneliness and social isolation

Community Activities

Activities are chosen with the individual—not for the individual. Depending on personal goals, interests, service authorization, and availability, SCC activities may include:

  • Visiting libraries, museums, parks, and recreation centers
  • Participating in arts, crafts, music, or educational activities
  • Attending local events, festivals, and community programs
  • Exercising, walking, swimming, or participating in fitness activities
  • Shopping for personal items or groceries
  • Practicing ordering and paying at restaurants
  • Learning to use public transportation
  • Exploring volunteer and community-service opportunities
  • Joining clubs, classes, or interest-based groups
  • Practicing appropriate social interactions
  • Planning and completing community outings
  • Learning about personal and community safety
  • Connecting with people who share similar interests

Our Person-Centered Approach

Every person deserves to have a voice in how they spend their time and pursue their goals. Before services begin, we take time to learn about the individuals:

  • Strengths and abilities
  • Interests and preferences
  • Personal goals
  • Communication style
  • Sensory and accessibility needs
  • Cultural background
  • Safety considerations
  • Desired level of independence
  • Individual service plan and authorized supports

We work collaboratively with the individual, family members, guardians, case managers, and other support team members to provide consistent, meaningful services.

Skills We Support

Community activities can provide practical opportunities to develop and strengthen:

Social and Communication Skills

Individuals can practice starting conversations, understanding social cues, expressing preferences, respecting boundaries, and interacting with others in different settings.

Independent Living Skills

Services may include practicing shopping, budgeting, scheduling, making choices, asking for help, managing personal belongings, and completing everyday community tasks.

Community Safety

Individuals can learn how to recognize unsafe situations, follow community rules, identify trusted helpers, protect personal information, and respond appropriately during an emergency.

Self-Advocacy

We encourage individuals to express their needs, make informed choices, establish personal boundaries, and participate in decisions that affect their lives.

Community Navigation

Individuals may practice following directions, recognizing landmarks, planning routes, using transportation, and locating helpful community resources.
